Community Action in Counties Manukau

Self Management Groups for Long term conditions

Day and evening winter workshops run throughout South Auckland to help people cope with longterm conditions including some sessions tailored specifically around Diabetes. Longtern conditions are any health conditions lasting 6 months or more. This includes common conditions such as diabetes, heart disease, depression, gout and lung disease, which if not managed properly can cause ill-health and even premature death.

Workshops are free and open to anyone with a longterm condition, including friends and family.

A number of organisations run specific modules for people with diabetes. They kick off by letting people know what diabetes actually is and how it affects the daily life of the person with diabetes. Experts are available to answer questions, such as: what can I eat? and why should I take my tablets? The supermarket tours are very popular and great for helping with the food shopping (including budget tips).

Or ask for a Self-Management Group referral from your doctor. If you have diabetes you can also ask your doctor about the 'Get Checked' Programme. Get Checked is a free annual diabetes check provided by practice nurses and general practitioners for people with diabetes.

 

 

 Get into Gardeing in Counties Manukau

Fruit and vegetable gardening is a great way to stay active and you can produce your own great healthy food. Community gardens and Teaching gardens are now operating in Mangere, Otara, Papatoetoe, Tuakau, Papakura.  Gardens are run through Community Boards, Councils, local schools and church groups.

Keep your family active with CM Active

Counties Manukau Active is a programme which encourages youth and families to make the change to a healthier more active lifestyle. The programme has a particular focus on low-socioeconomic areas. If you want to find out more about physical activites happening around the Counties Manukau region contact CM Active on 0800 Active or check out www.cmsport.co.nz

CM Active can provide a database of physical activites that are happening in the area, low cost training courses for volunteers to deliver physical activity programmes in targeted areas, basic activity equipment to allow programme delivery by volunteers and physical recreation and sports activities that lead to health and well-being.

Green Prescription

A green prescription is a way to support adults to become more active to benefit their health. Green Prescription support is a 3 month phone or txt support service to help adults get started and maintain regular physical activity. Support also involves providing information about appropriate and accessible activities as well as help with goal-setting. In Counties Manukau Green Prescription is delivered by Sport Auckland.

You can be referred to a Green Prescription programme in your area by asking your family doctor, nurse or health professional for a "Green Prescription."

Get Checked

Free visits are available to people with Diabetes once every 12 months. If you have Diabetes you should ask for your "Free Get Checked" visit when you make your appointment with your GP. Diabetes is a silent disease and serious damage can occur without any obvious pain or symptons. People need to show up to their doctor even if they are managing the condition and feeling well.
